The Physician’s Report can be downloaded and printed. That report has to be filled by the primary doctor (PCP), registered nurse (RN) or a certified doctor’s assistant.

Additional Requirements:

  1. Copy of the California ID/Driver’s License
  2. Copy of Insurance Card (s).
  3. Copy of Durable Power Attorney for Health Care.
  4. Most Current List of Medicines.
  5. Physician’s Report Current.
  6. Copy of Covid-19 Vaccine Record and Covid-19 Test Result not Older than 5 days.
